New clients will be prompted to complete intake paperwork electronically at the time of booking. Forms need to be completed prior to services being provided.

If you have or think you may have any contraindications, the required medical release form  can be downloaded here so it can be submitted to your doctor in order to be cleared to receive colon hydrotherapy.
